Attending Physician Statement (APS) is a medical report prepared by a doctor or hospital who previously treated you.The insurance company may request this if you have a past medical condition or treatment history that requires further clarification.

Step 2: Confirm declaration of non-existance medical conditions

You will require to fill-up the Declaration Form / Amendment to Proposal which represent a formal correction to the client’s health declaration to ensure the insurance records reflect the true medical condition.
